This presentation explores the complexities of combining pharmacotherapy and psychotherapy in psychiatric care, examining the challenges of achieving true integration, as opposed to simply adding modalities. We will consider implications of established treatment guidelines for when to use combined pharmacotherapy/psychotherapy as a first-line or second-line approach to psychiatric illness, as well as aspects of the evidence-based which provide guidance about how to select one treatment over the other for common psychiatric conditions. We will consider the intricacies of these two treatments may be additive, synergistic, or antagonistic, and how, particularly for complex patients, the two treatments may not only support one another, but may also be used defensively by patients and providers alike, in ways that undermine the effectiveness of one or both treatments. We will also explore how pharmacotherapy may influence treatment both alliance and patient defenses in ways that are either helpful or unhelpful to the psychotherapy. With this as a basis, we will consider how to optimize benefits of combining treatments and minimize interferences to the healthy use of one or both treatments. Participants will develop a deeper understanding of the technical, psychological, and systemic factors influencing integrated care and practical strategies for addressing these challenges in clinical practice.

  1. Define the difference between combined and integrated treatment and identify the competencies required for successful integration.
  2. Evaluate evidence-based criteria for selecting patients who may benefit most from concurrent psychotherapy and pharmacotherapy.
  3. Discuss the psychological and symbolic roles of medications in the therapeutic process and their effects on defenses and the therapeutic alliance.
  4. Analyze the benefits and challenges of split versus integrated treatment models in psychiatric care.
  5. Formulate practical strategies to improve communication and collaboration in interprofessional treatment settings involving split treatments.

David L. Mintz, MD, is a staff psychiatrist who joined the Austen Riggs Center in 1996. He presently serves as the director of psychiatric education, the associate director of training, and as a team leader. He also is principal investigator for the research initiative in Psychodynamic Psychopharmacology and author of the book Psychodynamic Psychopharmacology: Caring for the Treatment-Resistant Patient (2022, American Psychiatric Association Publishing). Dr. Mintz also coordinates the Elective in Psychodynamic Psychiatry at the Austen Riggs Center.
